For international physicians with an EU degree, the path to German medical licensure (Approbation) is legally streamlined through "automatic recognition". However, in practice, regional administrative speeds and specific requirements for the Medical Language Examination (FSP) determine how quickly you can actually start your career.  



1. Baden-Württemberg: The "Reform-Turbo"

Baden-Württemberg has positioned itself as one of the most efficient states for the recognition process.


  • Administrative Duration: With an average processing time of just 4 months for EU graduates, BW is currently the frontrunner in speed.  


  • Medical Language Examination (FSP): A major advantage in BW is its flexibility. Since the 2025/2026 reforms, private certificates such as FaMed are officially recognized alongside the traditional Medical Chamber exams. This allows applicants to completely bypass long waiting lists for exam dates at the Medical Chamber.  


  • Status: High efficiency due to increased staffing and digitized administrative processes.  


2. Hamburg: The Solid Middle

Hamburg offers a highly structured and modern procedure but maintains specific entry hurdles.


  • Administrative Duration: Similar to BW, the duration is approximately 4 months.  


  • Requirement: To apply in Hamburg, authorities usually require proof of a concrete job offer or residency within the city.  


  • Medical Language Examination (FSP): Recognition of private language certificates is less standardized than in BW and remains a case-by-case decision.  


3. Thuringia: Fair Exam, Long Wait

Thuringia is well-regarded among physicians for its "fair" exam atmosphere, but it currently struggles with significant staffing shortages.


  • Administrative Duration: EU graduates must currently expect a waiting period of about 8 months. For degrees from non-EU countries, the process can even take up to two years.  


  • Medical Language Examination (FSP): The FSP in Thuringia is considered particularly practical and fair, boasting a success rate of up to 99% with proper preparation.  


  • Challenge: A massive administrative backlog (over 650 pending cases at the end of 2025) has led to substantial delays across the entire process.  


Conclusion & Recommendation

Those who prioritize speed will currently find the best conditions in Baden-Württemberg, especially due to the recognition of private providers like FaMed. Hamburg is an excellent choice for doctors who already have a job offer in sight. Thuringia remains attractive for those who value a supportive exam atmosphere over a fast start and have the financial means to bridge the longer waiting period.
